If you have lost a stock certificate please be 100% sure that you cannot locate it. There is significant time and cost involved with replacing a stock certificate; therefore, it is to the benefit of both you and American Registrar that every step possible is taken to locate a misplaced certificate.

In order to replace a stock certificate American Registrar needs the following:

1.       You must notify American Registrar in writing that a certificate is lost. Click here for information on how to contact us.
         - The notification must be signed.
         - It must identify the company
         - It must include how the certificate is registered
         - Any other information that can be used to identify the certificate: certificate number, issue date, & the number of shares it
         represents.
         - Also include a fax number or a return mail address.
2.       After we have received notification that a certificate is lost we will send an information packet, either via fax or first-class
          mail, to you laying out the rest of the process.
